When you do a show at a club or bar, who pays royalty fees, if anyone does? Do you have to keep a play list for this purpose?

No KJ has to ever pay royalties. The royalties get paid out of the one-time cost of purchasing the CDG. In addition, the venue you're playing at has to pay an "entertainment license"... They're not even supposed to broadcast the radio, nor play a jukebox without paying for the license.
